These systems can eliminate the need for hand written records and manual data entry while enhancing the MAR by documenting precise administration times and making it easier for all caregivers to trace a patients care over time.23 They can also create a vital audit trail to facilitate follow up should an adverse event occur. In a related study 2 ; , the WHI reported quality-of-life measures collected at baseline and at 1 year in all women and at 3 years in a subgroup of 1511 women. Combined HRT had no meaningful effect on health-related quality of life. Among women 50 to 54 years of age with moderate to severe vasomotor symptoms, combined HRT improved vasomotor symptoms and resulted in a small improvement in sleep disturbance but had no effect on other measures such as vitality, mental health, depression, and sexual satisfaction. Recently, the WHI reported a slight but statistically significant increase in dementia among users of combined HRT 3 ; . In summary, the landmark conclusion of the WHI's findings is that combined HRT should not be used to prevent disease in postmenopausal women with intact uteruses. Hormone replacement therapy may still be indicated, however, to control menopausal symptoms. For women considering combined HRT for this purpose, the WHI provides the best measure of risks and benefits, and the current recommended practice is to prescribe the lowest effective hormone dose for the shortest possible duration 4 ; . Also, we determined that several of the drug samples were sent from locations that raise questions, such as from private residences. We also observed Internet pharmacies that obscured details about the drugs sold, such as other foreign pharmacies from which we ordered brand name drugs, but then received a generic or foreign version of the drug. Finally, about 21 percent of the Internet pharmacies that sent us samples were found to be under investigation by DEA or FDA. Reasons for the investigations included allegations of selling adulterated, misbranded, or counterfeit drugs and providing prescription drugs where no valid doctor-patient relationship exists. Nine of these pharmacies were from the United States, one from Canada, and four from other foreign countries. We provided a draft of this report to FDA, which generally agreed with our findings and conclusions. Our results do differ in one respect from a recent study in the Leuven-strain model of Vdr ablation.
